Payor and provider organizations identify and enroll consented individuals with specific risks that, if proactively managed, can lead to improved experiences, better health outcomes, and reduced cost. Examples include monitoring for improper inhaler use or noncompliance with medications for people living with asthma, elevated heart rate or fluid retention for individuals with congestive heart failure, increased risk for falls for elderly or frail individuals.
