Cherish Health develops advanced sensors and artificial intelligence combined with medical evidence and human touch to pioneer solutions that will help improve people’s lives and proactively enable their care where they live, work and play.
We improve the lives of elderly or frail individuals by enabling them to live independently at home and in supervised residential settings with more safety, companionship, and joy. We improve the lives of their friends and families by helping them more easily care for their loved ones and for themselves, and give them peace of mind while reducing hardship and cost for all involved.
We enable care for those living with long-term conditions by anticipating risks and then engaging proactively to help prevent avoidable deteriorations that might lead to ambulance rides, emergency room visits, and hospitalizations. We help those needing hospitalization to safely return home sooner, and even receive hospital-level care at home. In the process, we improve experiences while reducing workloads for care providers. This should reduce the cost of care for payors while maintaining – perhaps even improving – care outcomes.
Many people wish to live independently and remain in their homes well into their senior years. Yet they worry about what could happen if they have a fall or a medical challenge when alone. We monitor for falls, deterioration in mobility, and changes in activities of daily living, and send alerts and relevant guidance to care recipients, their loved ones, and/or other caregivers and healthcare professionals based on preferences and relevant care protocols.
Payor and provider organizations identify and enroll consented individuals with specific risks that, if proactively managed, can lead to improved experiences, better health outcomes, and reduced cost. Examples include monitoring for elevated heart rate for patients with congestive heart failure, increased risk for falls in elderly or frail individuals, and adherence to proper asthma medication use.
We developed Project Safe@Home in response to the COVID-19 crisis, and in a manner that applies to many public health emergencies. This turnkey solution enables continuous vital signs monitoring for infected individuals while they self-isolate where they live or in quarantine settings, It warns healthy people in front-line settings – armed forces personnel, first responders, clinicians, hotel and restaurant staff, and countless others – of possible exposure or infection so they quickly self-isolate before infecting others and get the care they need. The solution enables people to more safely return to work, and when integrated with contract tracing and antibody testing systems, becomes a key part of a community’s public health response strategy.
More active health monitoring with a focus on reducing avoidable and unnecessary admissions, especially those that can be prevented by predicting falls or other acute episodes.
Payor and provider organizations identify and enroll consented patients who have a high risk for readmissions post-discharge from hospitals, and for whom medical evidence indicates that with proper monitoring and engagement, such readmissions can be avoided and earlier return to baseline health and well-being can be achieved.
